Visitation Instructions

Dear Family and Friends of Good Shepards ALF,

It is with great relief that we announce the reopening of Good Shepards ALF on Tuesday, Sept. 15 to visitors. Please keep in mind that we are following a state mandate to allow ONLY LIMITED VISITATION at this time.

Your cooperation is very much anticipated and will be highly appreciated as we reopen safely and meaningfully not only to address the need to visit your loved ones but more importantly to safeguard the mental health and general well-being of our residents.

GENERAL INSTRUCTIONS:

  1. Please note that all registered visitors are subject to the approval of the resident and/or Power of Attorney (POA). Each resident is allowed five (5) registered “general visitors,” two (2) Essential Caregivers, and two (2) Compassionate Caregivers.[1] (Please read below to understand who qualifies as essential caregivers and compassionate caregivers) Only one (1) visitor will be allowed to visit at any given time.
  2. Click on the “Continue” button below to request visitation. Before submitting the ‘Visitor Information Registration Form’ make sure to read the ‘Visitation Guidelines’ and watch the 3 videos on Infection Control. This is mandatory.
  3. After we have received your request we will call you to schedule an appointment. Each appointment will be scheduled for 30 minutes at a time on a “First Come First Served” basis. Appointments are not 100% guaranteed. They may be canceled without advance notice depending on staffing and resident needs on the day of the appointment.
  4. Plan to arrive at least 20 mins before your appointment to allow yourself enough time to properly wear Personal Protective Equipment (PPE) that will include but not limited to gloves, masks, and gowns. Due to a limited supply, the facility will only provide one (1) set of PPE per visit. Only PPE provided by the facility will be acceptable for use. Please do not bring your own gloves, gowns, and/or facial covering. If you have a condition that prevents you from wearing any of the required PPE, please consult with the owners for possible alternatives.
  5. Once at the facility, please enter the side gate of the backyard. Then, proceed to the staging area where you will be donning your PPE. A separate e-mail will be sent confirming your staging area and where you will proceed to access the bedroom of your resident or another designated area once you have completed the registration. Make sure to fill out the visitor survey form and bring this with you as you enter the facility. Do not leave this form in the staging area. Please do not leave any valuables in the staging area. Good Shepards will not be responsible for lost or stolen items.
  6. Proceed to the secured access area where you will be met by one of the staff to let you in. Again, you will be given information on where to go from the staging area on a separate email. Please keep in mind that you must be wearing the proper PPE otherwise you will not be granted entry. Hand over the completed visitor survey with your name and date of visit. Your temperature will be taken and your information on the survey form will be reviewed just before you are allowed inside.
  7. Once inside the bedroom or designated visitation area, you will be asked to sit on a chair situated 6 ft. from the resident if you are a “general visitor.” Please always maintain distance throughout the visit if you are a “general visitor.”. Essential Caregivers and Compassionate Caregivers don’t have to social distance while performing their work. A staff member will assist you and the resident during the visit if a need arises. If you are a “general visitor,” please refrain from any physical contact to assist your resident. You may also call the facility number for immediate assistance if a staff member is not within reach during your visit.
  8. Upon completion of your visit, please doff or remove your gown inside the room by using the trash can provided inside the room or designated area. Masks and gloves must be removed outside of the bedroom and discarded in the trash cans provided outside or at the staging area.
  9. Please call the facility number to let staff know that you have left if they are not in the room with the resident or if you wish to communicate anything with them about your resident.
  10. Please consider writing a google review about your experience. We want others to learn from your experience and point them to our direction so we can share with them the same great care and attention your loved ones are receiving.

FREE COVID-19 Rapid testing for those who qualify for testing based on the guidelines.

1. Essential Caregivers(EC) are those who have been given consent by the resident or his or her representative to provide services and/or assistance with activities of daily living to help maintain or improve the quality of care or quality of life for a facility resident. Essential caregivers include persons who provided services before the pandemic and those who request to provide services. Care or services provided by essential caregivers must be identified in the plan of care or service plan and may include bathing, dressing, eating, and/or emotional support. EC’s are NOT required to social distance provided they wear the proper PPE and observe all pertinent infection control guidelines.

2. Compassionate Caregivers (CC) are visitors providing emotional support to help a resident deal with a difficult transition or loss, upsetting event, or end-of-life. Compassionate care visitors may be allowed entry into facilities on a limited basis for these specific purposes. CC’s are NOT required to social distance provided they wear the proper PPE and observe pertinent infection control guidelines.
